Students in grades 7-12 participating in school sports are required to have a record of a sports physical examination within the previous three years on file in the school office prior to the start of participation. A sports physical examination is needed every three years.
Summer is the best time for sports physicals especially if student athletes are playing a fall sport. A sports physical can be done in combination with an annual checkup or well-child visit. Let your healthcare provider know a sports physical is needed when you schedule your appointment.
It is recommended that the Minnesota State High School League Sports Qualifying Physical Examination form be used. Check with your provider if they have the form or you can get the form from your school office or on the athletics registration page of the school website.
Physical examinations are not required for participation in Fine Arts programs.
