  2. If you have more than one student, please fill out a form for each individual student, even if related to the same case.

  3. If you are a positive COVID case, or have been in direct contact with a positive COVID case please begin self-isolating if you are able. (For information on how to isolate within your home review the “Protect Yourself” section about halfway down the page.)

  4. A COVID Contact Tracer will be in contact with you 24-48 hours to gather more information, or with an email telling you your Return to Work/School date.

If you are in need of guidance outside of office hours, please contact the designated COVID contact listed above.  Someone will be checking emails and messages periodically on Saturday and Sunday.

   **IF this is a medical emergency please call 911.**

If you are SYMPTOMATIC with one “more common” symptom, or two “less common” symptoms and/or CONFIRMED POSITIVE - STAY HOME.  ALL other household members should STAY HOME as well. Isolate from other individuals within your home. If you are UNABLE to isolate, please let us know so we can determine the safest return date.

If a HOUSEHOLD MEMBER IS SYMPTOMATIC with one “more common” symptom, or two “less common” symptoms and/or CONFIRMED POSITIVE, ALL household members should STAY HOME. The symptomatic individual should ISOLATE from others in the household if possible.  If they are UNABLE to isolate, please let us know so we can determine the safest return date.

If you have been notified that you are a CLOSE CONTACT of a positive case that does not live within your household, the close contact MUST STAY HOME. Other household members not considered close contacts do not need to stay home. If the close contact becomes symptomatic, please refer to the list above "If you are SYMPTOMATIC...."

If you, or someone in your household, is tested due to having symptoms of COVID-19, ALL household members should STAY HOME until negative results are received.
